การประเมินการบริหารจัดการกองทุนหลักประกันสุขภาพในระดับท้องถิ่น องค์การบริหารส่วนตำบลทุ่งฝาย อำเภอเมืองลำปาง จังหวัดลำปาง / Assessment of Health Insurance Fund Management at the Local Level of Thoong Fai Sub District Administration Organization Muang Lampang District, Lampang Province

Abstract

การประเมิน นี้มีวัตถุประสงค์เพื่อติดตามการบริหารจัดการกองทุนระบบหลักประกันสุขภาพในระดับท้องถิ่นหรือพื้นที่ และเพื่อเสนอแนวทางแก้ไขและพัฒนากองทุนระบบหลักประกันสุขภาพ ในระดับท้องถิ่นหรือพื้นที่ องค์การบริหารส่วนตำบลทุ่งฝาย อำเภอเมืองลำปาง จังหวัดลำปาง เป็นการศึกษาเชิงคุณภาพ โดยใช้รูปแบบการประเมินแบบ CIPP เครื่องมือแบบสัมภาษณ์เชิงลึก  กลุ่มตัวอย่าง คือ  คณะผู้บริหารองค์การบริหารส่วนตำบลทุ่งฝาย และประชาชนในพื้นที่ตำบลทุ่งฝาย จำนวน 15 คน และการวิเคราะห์เนื้อหา

ผลการประเมินบริบทพบว่า ระบบกองทุนหลักประกันสุขภาพในระดับท้องถิ่นหรือพื้นที่ องค์การบริหาร ส่วนตำบลทุ่งฝาย มีความจำเป็นอย่างยิ่ง  หากพึ่งพางบประมาณของ อบต.  ทุ่งฝาย งานกิจการด้านสาธารณสุขคงไม่เพียงพอ  ระบบกองทุนฯ นี้มีประโยชน์กับชาวบ้าน  ตรงตามความต้องการของชาวบ้านในการดูแลสุขภาพ ทั้งที่เป็นเด็กเล็ก ผู้สูงอายุ ผู้พิการ หรือ คนที่ป่วยเป็นโรคเรื้อรัง                

ปัจจัยนำเข้าพบว่า  บุคลากรมีความพร้อม สถานที่ที่เอื้อต่อการปฏิบัติงานหรือการจัดกิจกรรม แต่อุปกรณ์ยังไม่เพียงพอต่อประชาชนในพื้นที่ ขณะที่ด้านงบประมาณมีเพียงพอ บางปีเหลืองบประมาณ  ส่วนกระบวนการพบว่า  การประชาสัมพันธ์ยังมีน้อย ความพร้อมและความเข้าใจบทบาทและหน้าที่ของคณะกรรมการบริหารกองทุนฯ ยังมีน้อย และควรได้รับการส่งเสริมและพัฒนาให้ความรู้ ความเข้าใจ ในการดำเนินกิจการหรือระบบกองทุน ด้านผลผลิต พบว่า กองทุนทำให้ประชาชนได้รับบริการสุขภาพ ตามที่กิจกรรมแต่ละโครงการกำหนดไว้นั้น  ด้านการมีส่วนร่วมในการบริหารและพัฒนาสุขภาพชุมชน พบว่า ประชาชนตระหนักมากขึ้น เข้าใจและเฝ้าระวังปัญหาสุขภาพพื้นฐานมากขึ้น แต่ในด้านการสร้างเสริมสุขภาพให้ร่างกายแข็งแรง ด้านการสร้างระบบการออกกำลังกาย ในชุมชน หรือ นวัตกรรมในการสร้างสุขภาพยังมีน้อยมาก กิจกรรม/โครงการยังคงเน้นให้เฝ้าระวังปัญหา ความดัน  เบาหวาน การค้นหาโรคแล้วรักษาสุขภาพ กิจกรรมการป้องกันโรคยังเกิดขึ้นไม่มากนัก

Abstract

The purposes were to track, manage health care, and to propose solutions, and to develop health care. It was a qualitative study by using CIPP Evaluation Model.The instruments used in the study were interviewed and group chat. The samples were 15 executives of Thoong Fai Sub District Administration Organization,and the analysis of previous interview. The results are summarized as follows:

The context results showed that “Assessment of Health Insurance Fund Management at the Local Level of Thoong Fai Sub District Administration Organization, is absolutely necessary. If it relies on only the budget from Thoong Fai Sub District Administration Organization, the health care system needed in the community is not enough funds. The Health Insurance Fund is useful to locals to meet the needs of residents in health care all are young people, old people, and the disabled or chronically ill people.

The input results was showed that the availability of personnel, facilities conducive to the practice or event is ready, but some equipment was minimal not enough for people in the area contributing to practice. While the budget is adequate, some years the budget was leftover. The process evaluation were found that lacks good publicity. While the results of ready and understand the role and functions of the Executive Committee of the Fund, and should be encouraged to learn. The product evaluation showed that funds had been made public health services. The event is scheduled for each project. The participation in the management and development of Community Health found that people become more aware understanding and monitoring the health problems more fundamental. But in the field of health to their body, the building exercises or innovation in creating healthy communities, there are very few. Activities / projects are continuing to focus on the problem of blood pressure, diabetes surveillance for the disease and maintain health. Activities prevention has not happened much.
